Dr. Luc Noel, Co-ordinator Blood Transfusion Safety, World Health Organization Secretariat The theoretical risk of vCJD transmission by blood transfusion was identified by the United Kingdom and other countries sharing a number of characteristics: public concern about transfusiontransmitted infections, effective prevention of established transfusion risks, objectives of maximal rather than optimal transfusion safety, nationally co-ordinated transfusion services, detection measures for BSE and vCJD and resources to implement precautionary policies. For several reasons, the risk is global. Blood donors who traveled may have been infected through the food chain in countries with BSE. In most countries, indigenous risk must be considered possible because of the potential for imported infected bovine material in human foodstuffs or because of imported or indigenous cattle fed infected MBM. Secondary transmission through transfusion could present broad dissemination of vCJD, and there could be human adaptation. Both would present a major obstacle to the global eradication of vCJD. Decisions about responses must balance the theoretical risks of transfusion against the risks presented by the safety measures. In addition, the balance must be economically, scientifically and publicly acceptable. Globally, there are 75 million blood donations per year. About 60% of those donations are used in developed countries, which collectively have only about 20% of the world's population. O BJECTIVES . We propose that Total Hip Replacement THR ; with correction of fixed flexion deformity of the hip and exaggerated lumbar lordosis will result in relief of symptoms from spinal stenosis, possibly avoiding a spinal surgery. A sequence of patients with this dual pathology has been assessed to examine this and suggest a possible management algorithm. M ETHODS . A retrospective study of 18 patients who presented with dual pathology was performed and the patients were assessed with regards to pre and postoperative symptoms, walking distance, and neurological status. R ESULTS . There were eleven patients with improvement in the spinal stenotic symptoms following hip replacement to an extent that none required spinal surgery. All of these patients had significant fixed flexion deformity at the hips. Two patients had spinal surgery before THR. Five patients had spinal surgery at varying lengths 6 months to 10 years ; following hip replacements as their spinal stenotic symptoms worsened over time, and at least three of these later group had lateral spinal stenosis on MRI. C ONCLUSIONS . In advanced hip osteoarthritis, a fixed flexion deformity may develop at the hip leading to an exaggerated lumbar lordosis in erect posture. In the presence of co-existing spinal stenosis, the exaggerated lumbar lordosis may worsen the spinal stenotic symptoms while standing and walking. Our findings suggest that when central lumbar spinal stenosis coexists with bilateral hip arthritis and fixed flexion deformity at the hip, THR should be offered first. We could not find any literature addressing this issue. Although it has been suggested in the literature that patients with spinal stenosis have a increased risk of neurological impairment following THR, we did not find any clear association.
