Dr. Luc Noel, Co-ordinator Blood Transfusion Safety, World Health Organization Secretariat The theoretical risk of vCJD transmission by blood transfusion was identified by the United Kingdom and other countries sharing a number of characteristics: public concern about transfusiontransmitted infections, effective prevention of established transfusion risks, objectives of maximal rather than optimal transfusion safety, nationally co-ordinated transfusion services, detection measures for BSE and vCJD and resources to implement precautionary policies. For several reasons, the risk is global. Blood donors who traveled may have been infected through the food chain in countries with BSE. In most countries, indigenous risk must be considered possible because of the potential for imported infected bovine material in human foodstuffs or because of imported or indigenous cattle fed infected MBM. Secondary transmission through transfusion could present broad dissemination of vCJD, and there could be human adaptation. Both would present a major obstacle to the global eradication of vCJD. Decisions about responses must balance the theoretical risks of transfusion against the risks presented by the safety measures. In addition, the balance must be economically, scientifically and publicly acceptable. Globally, there are 75 million blood donations per year. About 60% of those donations are used in developed countries, which collectively have only about 20% of the world's population. Developing countries severely lack access to blood transfusion therapies. The picture is even bleaker when the source of donations is examined. The safest donations are from voluntary, nonremunerated donors. Almost all the donations in developed countries are from such sources. In developing countries, well over half of donations are from family members or paid donors. The poorest countries rely almost solely on family replacement donations. Moreover, data in the WHO global database on blood safety indicate that 23% of countries do not test all blood donations for syphilis, 13% for HIV, 48% for HCV-antibody and 19% for HBs-Ag. Overall, the developed countries test 100% of the blood supply for the main transfusion-transmitted agents; in the developing countries, 43% of the blood is untested for those agents. In sum, 80% of the world has access to only 20% of the safe blood supply. Variant CJD and the global blood supply The WHO has been asked to advise on the global blood supply safety in relation to vCJD. There has been consensus see the Weekly Epidemiological Record of 24 November 2000 and 14 December 2001 at the WHO web site: who.int ; that the theoretical risk of vCJD transmission should not lead to depriving patients of vital transfusion therapy. It should not increase the established risk by recruiting new donors from populations previously considered unsafe. It should not divert resources from established risk prevention or other health care needs of the country. Those agreements led to six practical recommendations for optimal safety and the best use of resources. 1 ; Countries should strengthen the appropriate use of blood with consideration to known, unknown and theoretical risks. 2 ; Prevention of known risk is the priority. 3 ; Donor deferral and azacitidine. OR ; , which stains the DNA of cells. Cells were incubated with 1 Ag mL Hoechst dye and then visualized under a Zeiss-Jenalumar fluorescence microscope. Geller projects worldwide avonex sales of 6 million while wang estimates sales of 0 million and bacitracin. O BJECTIVES . We propose that Total Hip Replacement THR ; with correction of fixed flexion deformity of the hip and exaggerated lumbar lordosis will result in relief of symptoms from spinal stenosis, possibly avoiding a spinal surgery. A sequence of patients with this dual pathology has been assessed to examine this and suggest a possible management algorithm. M ETHODS . A retrospective study of 18 patients who presented with dual pathology was performed and the patients were assessed with regards to pre and postoperative symptoms, walking distance, and neurological status. R ESULTS . There were eleven patients with improvement in the spinal stenotic symptoms following hip replacement to an extent that none required spinal surgery. All of these patients had significant fixed flexion deformity at the hips. Two patients had spinal surgery before THR. Five patients had spinal surgery at varying lengths 6 months to 10 years ; following hip replacements as their spinal stenotic symptoms worsened over time, and at least three of these later group had lateral spinal stenosis on MRI. C ONCLUSIONS . In advanced hip osteoarthritis, a fixed flexion deformity may develop at the hip leading to an exaggerated lumbar lordosis in erect posture. In the presence of co-existing spinal stenosis, the exaggerated lumbar lordosis may worsen the spinal stenotic symptoms while standing and walking. Our findings suggest that when central lumbar spinal stenosis coexists with bilateral hip arthritis and fixed flexion deformity at the hip, THR should be offered first. We could not find any literature addressing this issue. Although it has been suggested in the literature that patients with spinal stenosis have a increased risk of neurological impairment following THR, we did not find any clear association. None developed foot drop following total hip replacement, although one patient did develop transient clawing of toes. O-177. Sealants Together with fillers like fine particles of soot medium molecular weight Oppanol B 10 SFN to B 15 SFN work very well as sealant in double glassing. Due to excellent gas barrier properties Oppanol inhibits either water gas as well as the insulation gas Argon to pass of the sealant of the double glassing. Oppanol B 15 SFN was found to be the optimised polymer regarding viscosity and very low fogging values for this application. The Oppanol types can also be used in formulations for elastomeric sealants in combination with fillers, oils and resins. The products Oppanol B 10 SFN to B 15 SFN, can be used in combination with the high molecular weight products in the range Oppanol B 50 SF 200 in pressure-sensitive adhesives mainly for medical applications. Oppanol B 10 SFN to B 15 SFN were also developed for the production of chewing gum base. They allow all chewing base manufacturers to stabilise their products in conformance with the pertinent local legislation. Based on their softness, elasticity, taste, neutrality and acceptance for fillers Oppanol types are preferred for the use in chewing gum base. The Oppanol B range can be blended with paraffin wax and other waxes and used to coat or laminate paper. Adding Oppanol B has the following benefits. Bitumen Higher melt viscosity Lower permeability to water vapour A more pliable, flexible film Better adhesion and barberry.
Prevention ; If you're exercising but not losing weight, measure your body fat. Q. I've heard that if you gain muscle, you will weigh more. Is that true? Can you help, because it's very discouraging? A. Often when starting a weight loss program, the scale will show an initial loss due to water weight and fluid loss. But many times the scale will start to go up even though your pants feel looser. The lesson is to focus on your body, not your weight. If you are working hard to get fit and see a weight gain, you may be adding calorieburning muscle! When determining your overall health, focus on your body fat ratio to lean muscle mass. A pound of muscle weighs the same as a pound of fat. But the body fat is more "fluffy" and the muscle is more "dense and compact." Muscle takes up less space in your body, so body weight may go up as you add compact, tight muscle mass.
