Concentrations of all compounds tended to level off after a few minutes, and the successive increases of the selected compounds transferred into the lavage fluid likely to be driven by osmotic forces may have been counteracted by proteolytic degradation within the airways. HA is an important connective tissue constituent, previously shown to be a sensitive marker of the matrix repair process occurring subsequent to inflammatory damage of the lung.31 The concentra tion of HA was higher in BAL fluid than in serum at the end of the lavage, and this may suggest either active secretion of HA to the ELF, a high local con centration of HA within the lung, or both, relative to that of circulating blood. A certain degree of de struction and remodeling of lung tissue may be pre dicted in long-term heavy smokers. Generally, how ever, our study favors the notion of a preserved function of the lung membrane in our smoking healthy subjects, since there were similar yields of all compounds, including HA, by lavage obtained from smokers and nonsmokers. Despite substantial dilution by the instilled saline solution, the lavage concentrations of ECP of about 20% of the serum concentrations in each of the aspi rated aliquots may suggest high local concentrations of ECP within the effective exchange area. The concentration-to-time curves of ECP rapidly formed a plateau, and this pattern of kinetic changes of ECP may suggest that equilibrium between the tissue compartment and the lavage fluid was reached very fast. Alternatively, saline solution itself or the phys iologic changes induced by the lavage, induced con tinuous emptying of ECP containing granules to the lining fluid. We conclude that because of the differences in diffusion rates between different compounds, none of the usual internal standards may be ideal as denom inators for quantitation of HA or ECP. The strong dependency of intraluminal dwell time of lavage fluid on the recovery of HA and other solutes, pinpoints the weakness of commonly used lavage techniques. With sufficient injury to the epithelium, it is likely that the kinetics of some of the compounds may change, and the bulk of those solutes in BAL may then originate to a larger extent from interstitium than from ELF. Generally, however, BAL could be useful in the description of the local defense of airway mucosa and in monitoring of the inflamma tory reaction in the lung, provided that the dilution markers are selected to match the inflammatory markers in terms of transfer rates. The kinetics of ECP in BAL, on the other hand, appears to be based on a mechanism other than transfer by osmosis. The relevance of the findings obtained by means of this method may still need to be proven in studies of various inflammatory lung diseases, because differ. The incoordination of the eliminating systems is the cause of those disturbances throughout the system." TREATMENT PLAN: 1. Sulphur, cream of tartar, Rochelle salts laxative compound 2. Massage COMMENTS: Poor eliminations and menopause were listed as contributing to headaches. 4015-1 FEMALE 40 3 23 ETIOLOGY AND PATHOPHYSIOLOGY: "As we find, the greater disturbance here is the after effect of an attack of intestinal flu that has produced inflammation through the alimentary canal, so that in and through the colon there are those tendencies for all foods taken to cause gas formations, giving suppressions to the activities of the organs of the central blood supply and nerve supply; affecting at times the lungs, heart, liver and kidneys. The background report stated: "The trouble has been diagnosed as toxic hyperthyroid, yet treatment for same does not relieve the terrible sick-headaches which occur each month or sometime oftener." 3900-1 FEMALE 33 3 25 ETIOLOGY AND PATHOPHYSIOLOGY: "As we find, there are conditions that cause a great deal of anxiety and disturbance to the body functions at times, upsetting the body during the periods and causing distress especially through the duodenum and portions of the jejunum; producing headaches at such periods, easy tiring of the body, and the very poor eliminations or tendencies towards aggravations throughout the colon areas."
