Concentrations of all compounds tended to level off after a few minutes, and the successive increases of the selected compounds transferred into the lavage fluid likely to be driven by osmotic forces may have been counteracted by proteolytic degradation within the airways. Generally, how ever, our study favors the notion of a preserved function of the lung membrane in our smoking healthy subjects, since there were similar yields of all compounds, including HA, by lavage obtained from smokers and nonsmokers. Despite substantial dilution by the instilled saline solution, the lavage concentrations of ECP of about 20% of the serum concentrations in each of the aspi rated aliquots may suggest high local concentrations of ECP within the effective exchange area. The concentration-to-time curves of ECP rapidly formed a plateau, and this pattern of kinetic changes of ECP may suggest that equilibrium between the tissue compartment and the lavage fluid was reached very fast. Alternatively, saline solution itself or the phys iologic changes induced by the lavage, induced con tinuous emptying of ECP containing granules to the lining fluid. We conclude that because of the differences in diffusion rates between different compounds, none of the usual internal standards may be ideal as denom inators for quantitation of HA or ECP. The strong dependency of intraluminal dwell time of lavage fluid on the recovery of HA and other solutes, pinpoints the weakness of commonly used lavage techniques. With sufficient injury to the epithelium, it is likely that the kinetics of some of the compounds may change, and the bulk of those solutes in BAL may then originate to a larger extent from interstitium than from ELF. Generally, however, BAL could be useful in the description of the local defense of airway mucosa and in monitoring of the inflamma tory reaction in the lung, provided that the dilution markers are selected to match the inflammatory markers in terms of transfer rates. The kinetics of ECP in BAL, on the other hand, appears to be based on a mechanism other than transfer by osmosis. The relevance of the findings obtained by means of this method may still need to be proven in studies of various inflammatory lung diseases, because differ.
