Over the past year, we've experienced incredible growth in the number of physicians who have joined our editorial board as editors-in-chief, section editors, deputy editors and authors. We've added over 400 contributors from leading academic institutions in our continued efforts to expand the content and increase the utility of UpToDate as a clinical resource. We're excited to welcome a number of editors-in-chief to the leadership of our specialties in development, with an all-new team for Emergency Medicine and a third co-editor-in-chief for Allergy and Immunology. We've also expanded the leadership of two of our existing specialties. Three new editors-in-chief have joined Dr. Burton Rose to head up our Nephrology specialty. Additionally, six new editors-in-chief have taken the lead in Family Medicine following Dr. Larry Culpepper's departure. Dr. Larry Culpepper, who joined UpToDate in 2000 as editor-in-chief, was instrumental in the development of this specialty and we would like to thank him for his significant contributions. Additionally, over 100 prominent physicians have joined us as section editors across our specialties. These physicians will work closely with us to recruit new authors and have a pivotal.

Myofibroblasts express -smooth muscle actin and the ED-A isoform of fibronectin and are phenotypically intermediate between smooth muscle cells and fibroblasts 20, 21 ; . In pathological lesions, myofibroblasts may arise from the transdifferentiation of epithelial cells 22 ; or the differentiation of stromal fibroblasts under the influence of profibrotic growth factors, such as TGF, particularly TGF- 1. TGF- 1 has been shown to induce myofibroblastic differentiation of fibroblasts both in vivo and in vitro and is implicated in the progression of most forms of human fibrosis 4, 14, 16 ; . The present study describes for the first time a change in HA turnover as a result of the differentiation of fibroblasts to myofibroblasts. The most obvious feature of this change was accumulation of HA in the medium, on the cell surface, and associated with the cell layer matrix extract. HAS 2 mRNA expression was, however, lower as a result of myofibroblastic differentiation. This was in contrast to the reported effect on epithelial-mesenchymal transformation induced by adenoviral infection with HAS 2. In the current study it appeared, therefore, that the changes in HA.

Were also determined on a per-compound basis. The weighting system used was the same as that used in the calculation of the plasma concentration wMLFE. There was considerable overlap in the compounds comprising the training sets for the four different methods. The results of this analysis for the individual methods and their respective training sets, as shown in Table 2, demonstrate that the PBPK model apparently compares favorably with the Ward and Smith 2004 ; method, with the former having a lower average -fold error and predicting a slightly greater proportion of compounds within a factor of 3 of the observed values. However, they also suggest that the current version of the PBPK model might be somewhat less accurate than the methods of Caldwell et al. 2004 ; in the quantitative prediction of human clearance. To further investigate the relative capabilities of the four methods, a similar statistical analysis of their predictions of clearance in humans for the PBPK model test set compounds was carried out, and is summarized in Table 3. This test set was completely independent, so that the compounds selected for inclusion were not present in any of the training sets, but also represented those for which suitable rat and human in vivo intravenous dosing data were readily available. Predictions of human clearance made by the Caldwell et al. 2004 ; and Ward and Smith 2004 ; methods were calculated from published values of rat in vivo clearance for the test set compounds and evaluated against the corresponding published human clearances. Again, the statistics for the PBPK model were derived from the weighted mean -fold prediction error for each compound. The predictive accuracy of each method, and particularly of the Ward and Smith 2004 ; method, is generally poorer for the test set compounds than for those comprising the respective training sets, but still at what might be considered an acceptable level for the early stages of drug discovery Caldwell et al., 2004; Ward and Smith, 2004 ; . However, the deterioration in overall performance is proportionately less for the PBPK model than for the other methods. Con. These investigators postulated that, since coronary atherosclerosis is diffuse, plaques might be expected to develop in a multifocal pattern, resulting in multiple complex plaques in different locations. Any one of these lesions might progress to total occlusion and MI. This study was designed to document the incidence of multiple complex plaques in patients with acute transmural MI, and to determine their influence on outcomes. Conclusion: Patients with acute MI may harbor multiple complex plaques that are associated with adverse outcomes. Plaque instability may be a widespread process throughout the coronary arteries. STUDY 1. Analyzed angiograms of over 250 consecutive patients for coronary plaques characterized by thrombus, ulceration, plaque irregularity, and impaired flow. 2. Lesions were considered complex if they caused at least 50% stenosis and had two or more morphologic features: filling defects; plaque ulceration; plaque irregularity; and impaired flow. Patients with multiple complex plaques were less likely to undergo primary angioplasty, and to more commonly require bypass surgery. They also had greater depression of left ventricular function. 3. During the following year, multiple complex plaques were associated with an increased incidence of acute coronary syndromes, repeat angioplasty, and bypass surgery. DISCUSSION 1 "The presence of coronary plaques with complex morphologic features is the angiographic hallmark of unstable coronary syndromes, and correlates with pathologic plaque rupture and thrombus." Progression of stenosis and clinical instability are characteristic of such lesions. There is a striking association between complex morphologic features and clinical instability. Patients with acute MI may harbor multiple complex coronary plaques which are associated with adverse clinical outcomes. 2. This . "supports the concept that plaque instability is not merely a local vascular accident, but probably reflects a more generalized pathophysiologic process with the potential to destabilize atherosclerotic plaques throughout the coronary tree". 3. Hamback, P. A., Oksanen, L., Ekerholm, P., Lindgren, A., Oksanen, T. and Schneider, M. 2004. Predators indirectly protect tundra plants by reducing herbivore abundance. Oikos 106: 85 92. The role of predators in controlling herbivores and indirectly affecting plant abundance is controversial, and some have argued that such trophic cascades are rare in terrestrial habitats. To examine the potential of trophic cascades in a shrubby tundra ecosystem, vole densities, plant damage and plant cover were examined in areas with and without small mammal predators. On islands without predators, vole densities and plant damage were upto five times higher compared with predator-rich mainland habitats. As a consequence, the abundance of three out of five dwarf shrub species was substantially reduced on predator-free islands, providing evidence for strong cascading effects in a rather large-scale terrestrial system. Herbs on the other hand were not affected by the increased herbivory on islands. Researchers at Harvard found that more than 90 percent of the 3, 300 women who developed diabetes over a 16-year study period were overweight, inactive, and smokers.
