Icodextrin is a glucose polymer obtained from starch hydrolysis. It is used as an osmotic agent at 7.5% for peritoneal dialysis PD ; . Its use in PD has been associated with several side effects separate from the one reported here, the most frequent being sterile peritonitis. Recently, three mechanisms have been proposed to explain the occurrence of sterile peritonitis: allergy to dextrin, production of anti-dextran antibodies, and impurities introduced during manufacture. Here, we report a peritoneal mononucleosis outbreak that is highly suggestive of being a consequence of the lastmentioned mechanism. During the period December 2001 to May 2002, a group of 8 Spanish hospitals whose individual PD programs regularly share information and activity reported 29 cases of sterile peritonitis associated with icodextrin use in continuous ambulatory peritoneal dialysis CAPD ; patients [mean age: 60.7 14.47 years; 8 women 27.59% ; , 21 men 72.41% mean time on PD: 25.21 35.31 months; mean time on icodextrin: 15.17 11.03 months]. Of the 29 patients, 51.8% showed no symptoms. The remainder presented with mild abdominal discomfort and anorexia. Only 2 patients showed general malaise, severe nausea, fever, and abdominal pain. This finding indicates that a declining T cell population is not responsible for inducing the virus to infect monocytes more prevalently during therapy in our study subjects. The phylogenetic analysis for each patient suggests that monocytes may play a role in the rebounding viral population after the cessation of therapy, as it was shown to provide replication-competent virus to the plasma during and after therapy. This was seen in both patients 1180 and 1184, where the plasma virus was shown to cluster closely to both CD4 T cells and monocytes directly following therapy, as well as in patient 1396 where plasma virus sampled just after stopping therapy contained some sequences with homology to both CD4 T cells and monocytes. HIV-1 infected CD14 monocytes, after circulating in the peripheral blood for 1 to 3 days, may differentiate into macrophages and disseminate into various tissues. HIV-1 within these tissue macrophages then replicate, and some preferentially infect blood monocytes again [1216]. This circular pathway involving monocyte-macrophage-selected HIV-1 strains may allow for HIV-1 in monocytes macrophages to become a separate compartment during therapy [1216]. However, no results directly support this hypothesis. Furthermore, in vitro studies have demonstrated that fresh monocytes are highly resistant to HIV-1 infection [40 44]. Although recent studies indicate that HIV-1 replication occurs in blood monocytes in vivo, it remains unclear as to what extent HIV-1 can infect and replicate in blood monocytes in vivo [1216]. The data presented here indicate a possible role for monocytes macrophages as a separate drug sanctuary in the blood, which in turn may affect the evolution of drug-resistant mutations and the overall persistence of HIV-1 in the body during therapy. Limitations of the present study include a lack of a large patient sample size, shortage of sequences for analysis due to technical difficulties for some time points, and only a few plasma time points studied. While further studies with a larger cohort and more intensive sampling should be conducted in order to confirm the findings of the present study, our findings indicate that the creation of better drug regimens may be necessary to combat HIV-1 infection.

As noted, the incidence of pertussis is increasing despite the availability of effective diphtheriatetanus-acellular pertussis DTaP ; vaccines.7 Of primary importance to preventing the occurrence of pertussis is continued vigilance to maintain high levels of vaccine coverage, including the timely completion of the pertussis vaccination schedule. The current recommendation for pertussis vaccination consists of a primary series of DTaP, which includes 3 doses given at 4- to 8-week intervals beginning at 6 to weeks of age, with a fourth dose given 6 to 12 months after the third dose typically at approximately 15 to 18 months of age ; .16 A fifth dose is administered in children who received all 4 primary doses prior to their fourth birthday. Despite these guidelines, coverage rates with 4 doses of DTaP remain below that of other vaccines, with the exception of varicella and pneumococcal vaccines.
